Responsibilities:

  • Manage a caseload of newly admitted patients, providing individual and group counseling services. Conduct intakes, discharge planning, needs assessments, and treatment plans within regulatory timelines.
  • Engage patients in their recovery process through collaborative treatment planning.
  • Perform daily follow-ups with new patients via in-person or telehealth as clinically appropriate.
  • Monitor and document patient progress, identifying relapse risks early.
  • Provide case management and coordinate care with external providers.
  • Participate in weekly case conferences and clinic management meetings.
  • Evaluate patients for high-risk conditions and implement Clinical Risk Management policies.
  • Maintain accurate and compliant documentation using internal protocols and evaluation tools.
  • Provide make-up counseling sessions for missed appointments as needed.
  • Attend all required training and maintain professional development.
  • Other duties as assigned.


Requirements:

  • Associate degree in Human Services or related field, or completion of the majority of approved counselor certification courses. Note: Kern County requires a bachelor's degree or 75% of required certification coursework.
  • 2–5 years of experience in substance use disorder treatment or related field.
  • Counselor registration/certification with a CA-recognized certifying body (CCAPP, CADTP, ACCBC).
  • Compliance with CEU/CEH requirements as determined by regulatory agencies. Knowledge of addiction treatment, patient evaluation, and counseling techniques.
  • Strong communication, time management, and organizational skills. Proficiency in Microsoft Office and EHR systems.
  • Completion of initial counselor trainee training.


Preferred Education and Certifications Education: Associate degree (required) bachelor's degree (preferred) in Human Services or related field


Certifications (Preferred): LCAC LAC CDAC CSAC
