Case Manager

A Case Manager is responsible for providing resources, advocacy, and direct support to address barriers related to housing, healthcare, behavioral health, substance use, and other social determinants of health. This role requires strong collaboration with internal programs and external community-based organizations to ensure clients receive coordinated, holistic care. This Case Manager role will be assigned to the shelter and provides comprehensive, trauma-informed case management services for adult residents and their dependents in OSP's emergency shelter. This position focuses on helping clients stabilize during their stay and prepare for their next steps. In addition to technical skills, a Case Manager must demonstrate empathy, effective communication, and the ability to build and maintain trusting relationships with clients who may be in crisis or transition. A deep understanding of trauma-informed practices and the ability to navigate complex client needs will be essential for success in this role.

Duties and Responsibilities:

• Complete thorough Client Needs Assessments for all new shelter residents following their initial Client Services intake.
• Conduct ongoing assessments, safety planning, and development of individualized Care Management plans focused on stabilization and preparation for next steps (ECM enrollment, Transitional Housing, or other long-term supports).
• Actively promote client empowerment, independence, and self-advocacy within a structured shelter environment.
• Provide education and support as needed, including topics such as domestic violence, sexual assault, parenting, substance use, coping skills, and life skills development.
• Maintain timely, thorough, and accurate case notes and documentation via the organization's case management system.
• Ensure all informed consents are completed and protocols for confidentiality and information sharing are followed.
• Provide comprehensive case management, including referrals to internal and external services, and coordination of transportation to ensure attendance at critical appointments.
• Collaborate closely with the internal team members and external partners to identify and remove barriers to stable housing and long-term care engagement.
• Present case information at multidisciplinary staffing meetings to ensure coordinated care and continuity between shelter and ongoing case management programs.
• Participate in case staffings and regular supervision to assess client services, shelter operations, and program outcomes.
• Other duties as assigned.

Knowledge, Skills and Abilities:

• Deep understanding of trauma-informed care principles and best practices in supporting individuals and families experiencing homelessness or housing instability.
• Proven ability to manage complex cases and navigate multiple service systems effectively while demonstrating leadership, initiative, and sound judgment.
• Strong computer and documentation skills; proficient in Microsoft Word, Outlook, and other case management or EHR systems.
• Excellent problem-solving skills to manage crisis situations and collaborate with a multidisciplinary team in a dynamic shelter setting.
• Demonstrated ability to work effectively with culturally diverse populations and to implement best practices that promote equitable outcomes and respect for individual experiences, cultures, and identities.
• Comprehensive knowledge of community resources, including housing, healthcare, behavioral health, and social service systems, and the ability to coordinate effectively across these networks.
• Strong communication skills, both written and verbal, with the ability to build trust and rapport with clients who may be experiencing trauma or transition.
• Exceptional time management and organizational skills to balance a dynamic caseload within a structured, fast-paced environment.
• Valid California driver's license and access to reliable transportation.

Education and Experience:

• BA or BS degree in social work, human services, psychology, or related field preferred; or a minimum of three year experience working with survivors of domestic violence, sexual assault, and/or human trafficking.
• Basic knowledge and ability to work in computer programs (i.e., Outlook, Microsoft Suite).
• Utilizes trauma-informed care and survivor-centered practices when working with clients.
• Satisfactory completion of Domestic Violence and Sexual Assault Counselor Training pursuant to California Evidence Code (E.C.) 1037.1 and 1035.2 (post-hiring requirement) and ongoing completion of continuing education hours to maintain state certification.
• Ability to pass a background investigation including fingerprinting.

Physical Requirements:

• Ability to navigate the shelter facility and assist clients in various areas, which may include walking up and down stairs or moving between offices.
• Capability to sit for extended periods while conducting interviews or paperwork, as well as standing for various tasks such as providing support or assistance.
• Ability to lift and carry items up to a certain weight (e.g., up to 25 pounds), which may include files, supplies, or assistance for clients if needed.

Rate of Pay:

Full Time, Non-Exempt

$22.00 - $30.00 per hour DOE
