Responsibilities

• Provide skilled nursing care to patients in home health settings
• Assess patient conditions and develop individualized care plans
• Administer medications, wound care, and treatments as prescribed
• Educate patients and families regarding disease management and home care
• Monitor patient progress and communicate updates to physicians and care teams
• Maintain accurate and timely patient documentation
• Coordinate care with interdisciplinary healthcare professionals
• Ensure patient safety, comfort, and quality care during home visits

Requirements

• Active RN License in California
• Minimum 2 years of Home Health nursing experience
• BLS Certification required
• Valid driver's license and reliable transportation preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently and manage patient schedules effectively
