WORK YEAR: Full-time / 11 Months / 207 Days (prorated based on date of hire) /7 hours per day
START DATE: As soon as possible after interview
SALARY: Schedule 5, Range 22, $20.84-$30.80 an hour. Placement dependent on experience.

The Transition Specialist I provides information to advise students on issues and concerns related to high school completion and prerequisites for postsecondary programs, acts as liaison with students, parents, school staff and community partners regarding students’ education plans, facilitates and coordinates resources for enrollment in high school, postsecondary programs or career options. The Transition Specialist I also assists students in developing daily living skills and ways to access community and parental support services.
Transition Specialist I’s are distinguished by their high level of responsibility and decision-making in working with various student populations, including at-risk/high-risk and those with special needs, who need services necessary to successfully transition into school, community, college, and/or employment. These services include education, college and career readiness and training, and developing daily living skills.
EXAMPLE OF DUTIES: [May Include, but is not limited to the following.]
- Assists and supervises students in classroom during academic activities
- Models appropriate behavior; maintains morale to motivate transitioning students; reinforces appropriate behavior modifications as designated by classroom teachers
- Evaluates student progress toward educational goals and graduation requirements
- Operates standard office and classroom equipment and technology
- Maintains awareness of surroundings, safety needs, physical and emotional changes in students; reports concerns to staff
- Attends in-service and Professional Learning Community meetings as required
- Supports credentialed staff with administration of new student assessments and orientation processes and assists in developing student’s education plan and transition portfolio and other program-related-activities
- Serves as a liaison for students, families, school districts and other related agencies involved in the transition process; provides information and support; assists students in completing necessary forms and obtaining documents for transition, training, education and employment; provides interventions when necessary
- Participates in and/or facilitates collaborative meetings. Attends IEP meetings, with staff and agencies to provide information and recommendations
- Develops systems for collecting and comparing student data; maintains accurate records and reports including case notes, charts of student progress, school transcripts, intake/orientation files, records and various related forms
- Develops and maintains a professional rapport with students, staff and other public agencies
- Demonstrates sensitivity to needs of students and staff and maintains confidentiality of information regarding students

- Support academic and vocational assessments
- Respond quickly in emergency situations
- Understand and maintain confidentiality of student information
- Perform general clerical duties including recordkeeping and filing
- Use a computer to maintain various databases, create certificates and flyers, and conduct online research
- Use standard office and classroom equipment
Experience and Training:
- Bachelor’s degree or 96 semester units from an accredited college or university with coursework in Social Sciences, Psychology, Education, Criminal Justice or related fields
- Experience instructing/facilitating groups of children and/or young adults
- Experience working with various student populations, including at-risk/high-risk and those with special needs
- Any combination of training and experience which demonstrates ability to perform the duties and responsibilities as described
